9/28/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Client appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C informed him this session was going to focus on his ISP goals and progress made towards them. Client reported to QMHP-C he continues to get in trouble with his mother because he does not listen. 2. Behavior: Client displayed hyperactive behaviors evident by him starring out the window not being engaged in the session. 1.Intervention: QMHP-C began the session by inquiring about Client emotional well-being. QMHP-C reviewed Client’s behavior at home and if he had experienced any symptoms of anger and anxiety at home. QMHP-C assisted Client in expressing how he was behaving at home when asked to do something or not getting his way. QMHP-C used active listening and supportive statements to encourage Client as he shared his thoughts on how he was treating his sibling. 2. Intervention: QMHP-C discussed with Client some of the triggers he feels make him upset and angry. QMHP-C used active listening and supportive statements to support Client while expressing himself about being asked to do something or not getting his way. QMHP-C used suggestions to assist Client with understanding why he needed to follow directions and that he will always have to follow someone’s directions in life. 1.Response: Client was receptive to QMHP-C's suggestions for today's session. Client was able to follow directions and listen to the QMHP-C suggestions. Client was able to answer questions and express why he has been more respectful and appreciative of QMHP-C’s feedback, evidenced by his "I realize you care and only want me to do better. 2. Response: Client was able to participate and discuss his triggers. He expressed that he enjoyed hanging with the QMHP-C and learning ways to not get so angry and mean, evidenced by "I think that when you make me explain how I am feeling, I sometimes realize that I am actually not mad but feeling something else", comment. Client was able to participate in therapeutic conversations with the QMHP-C on not saying mean and negative things. QMHP-C discussed the session with Client’s father and asked him for some support to encourage Client to follow directions and do what he was being asked.
